At SRK Care at Home, our post-hospital care services help seniors age 65 and older recover safely at home after surgery, illness, injury, or hospitalization. The first weeks after discharge carry the highest risk of falls and readmission for older adults. Our non-medical caregivers provide daily support with personal care, mobility assistance, meal preparation, medication reminders, light housekeeping, transportation to follow-up visits, and wellness checks. This is non-clinical support that complements, rather than replaces, skilled nursing or home health care. We also support eligible seniors through Maryland Medicaid, LTSS, In-Home Aides Services, and Home and Community-Based Services.
