At SRK Care at Home, our post-hospital care services support children ages 2–18 recovering at home after surgery, illness, injury, or a hospital stay. Our caregivers assist with personal care, feeding, mobility, supervision, medication reminders, transportation to follow-up appointments, and getting back into normal school and home routines. We work closely with parents and guardians, following your instructions and the discharge plan, and reporting any change in your child's condition promptly. We also support eligible families through Maryland Medicaid, LTSS, In-Home Aides Services, and Home and Community-Based Services.
